We just completed a brand new complete build. Everything is working except one of three fans on the graphics card. The GPU is Ry5600XT. What do you think we might have done wrong or could it possibly be a faulty GPU?

The fans are thermally controlled. Have you put the card under heavy load to see if the third fan spins? -
No, I'm afraid to do that. Could it be that only 2 come on at start up and the third only comes on at a particular temp?

It's very possible, they're probably all off when you're idle on your desktop. I would use a tool like MSI Afterburner to monitor your temperatures. Load up a game or benchmark and test it.
